Global Dengue Challengue-A Growing Global Public Health Threat

Dengue is now endemic in 128 countries, with WHO estimating approximately 400 million DENV infections and around 100 million symptomatic cases annually. The disease continues to place pressure on healthcare systems, particularly during outbreaks and periods of increased transmission.

The challenge extends beyond the number of infections:

Matching the Right Test to the Right Time

WHO’s Laboratory Testing for Dengue Virus: Interim Guidance, April 2025 emphasizes that dengue diagnostic strategies should be adapted according to the stage of illness, local epidemiology, circulating orthoflaviviruses and laboratory capacity. WHO notes that there is no single universal diagnostic algorithm applicable to every setting.

-Acute Phase-Approximately Day 1–7

Preferred direct detection approaches include: DENV RNA by NAAT / RT-PCR,  or NS1 Antigen  by ELISA / Rapid Diagnostic Test.These methods support early detection when viral RNA and NS1 antigen are more likely to be detectable.

-Later Stage - After Day 7

IgM antibody testing becomes increasingly important for identifying recent dengue infection. Additional serological investigation, including paired samples and evidence of seroconversion where appropriate, may be required when initial results are inconclusive.

-When Differential Diagnosis Is Needed

Negative dengue results do not necessarily end the diagnostic investigation. WHO recommends considering other arboviruses and febrile diseases, particularly where Dengue, Zika, Chikungunya or other orthoflaviviruses co-circulate.
