From Rapid Screening to Molecular Answers: Building a Flexible Respiratory Testing Strategy

A patient has a fever, cough, or sore throat.

At home, they may want a quick answer.

At a pharmacy or primary care setting, the priority may be rapid assessment and triage.

In a hospital, clinicians may need to know exactly which pathogen is involved.

Same symptoms. Different questions. Different testing needs.

That is why respiratory testing should be flexible—not built around a single technology.

Rapid Testing for Fast, Accessible Answers

For self-testing, screening, and near-patient triage, rapid antigen tests can provide a simple and accessible first answer.

Macro & Micro-Test offers three respiratory combinations:

5-in-1
COVID-19 / Flu A&B / RSV / Adenovirus

3-in-1
COVID-19 / Flu A&B

2-in-1
RSV / Adenovirus

Fast results and straightforward workflows make these formats suitable for decentralized testing where timely initial information matters.

Molecular POCT When More Information Is Needed

Sometimes the clinical question is broader:

Which respiratory pathogen is involved?

A wider pathogen differential may be relevant, particularly when symptoms overlap or when clinicians need more specific pathogen information to support patient management.

This is where molecular POCT can add value.

Running on the Eudemon™ AIO800, Macro & Micro-Test offers two lyophilized respiratory molecular assays:

HWTS-RT177
RSV • Adenovirus • hMPV • Rhinovirus • Parainfluenza Virus I/II/III • Mycoplasma pneumoniae

HWTS-RT76
Influenza A • Influenza B

Sample processing, nucleic acid extraction, fluorescence PCR amplification, and result interpretation are integrated into one automated Sample-in-Result-out workflow.

One Respiratory Season. Different Testing Needs.

The right testing approach depends not only on where testing happens, but on the question that needs to be answered.

Why Flexibility Matters

Respiratory viruses do not follow one single pattern.

ECDC’s current EU/EEA framework for integrated respiratory virus surveillance covers influenza, SARS-CoV-2, RSV, and other respiratory pathogens, reflecting the need to monitor respiratory disease continuously and adapt surveillance as epidemiology changes. ECDC also notes that SARS-CoV-2 does not behave as a strictly seasonal winter virus and that influenza or RSV activity can occur outside expected seasonal periods.

Recent surveillance also illustrates how different pathogens can peak at different times. In the U.S. 2025–2026 season, influenza hospitalization peaked in late December 2025, COVID-19 in early January 2026, and RSV in late February 2026.

For respiratory testing, this means one fixed testing strategy may not be enough for every situation.

From “Antigen or Molecular?” to “When Do We Need Which?”

The goal is not to choose one technology for every patient.

It is to match the testing approach to the need.

Rapid antigen testing helps provide fast, accessible initial answers in decentralized settings.

Molecular POCT provides broader and more specific pathogen information when differentiation matters.

Together, they create a more flexible respiratory testing portfolio—from rapid screening to molecular identification.
